SANCTUARY CHURCH TOUGH MUDDER WAIVER & RELEASE OF LIABILITY

READ CAREFULLY — THIS IS A LEGAL DOCUMENT

By signing below, I acknowledge and agree to the following:

1. Voluntary Participation: I am voluntarily participating in the Tough Mudder event hosted by Sanctuary Church. I understand that this is a physically demanding and potentially hazardous activity involving risks such as slips, falls, collisions, dehydration, exposure to elements, and obstacles that may cause injury or illness.

2. Assumption of Risk: I understand and accept all risks associated with this Event, whether known or unknown, including but not limited to physical injury, property damage, or death. I confirm that I am physically fit and capable of participating safely.

3. Medical Treatment: I authorize Sanctuary Church to provide first aid and/or secure emergency medical care if necessary, at my expense, and release them from any liability related to such care.

4. Release of Liability: I hereby release, waive, and discharge Sanctuary Church, its pastors, volunteers, staff, affiliates, sponsors, and property owners from any and all claims, demands, or causes of action arising out of or related to my participation in this Event.

5. Photo/Video Release: I grant permission for photos and video taken during the Event to be used by Sanctuary Church for promotional and ministry purposes, without compensation.

6. Minors: If the participant is under 18, a parent or legal guardian must sign below, accepting full responsibility and agreeing to all terms of this waiver on the minor’s behalf.

By filling out the form below, I certify that I have read and understood this waiver, and agree to its terms freely and voluntarily.

REQUIREMENTS: CLOSED-TOED SHOES (No Crocs, Hey-Dudes, Slides, Sandals)
